Behavior Agreement
We want every participant to feel safe, respected and free to try things. By signing, the participant and parent/guardian agree that the participant will:
Treat other participants, instructors and volunteers with respect and kindness.
Follow all safety instructions at all times as asked by the instructors.
Use tools, materials and equipment only as directed, and ask when unsure.
Take care of shared supplies and the workspace, and help clean up.
Stay in the designated workshop areas and with the group.
Engage actively in all activities and contribute positively to the learning community. No cell phones are allowed during the sessions unless and otherwise directed by the coach for educational purposes only. Follow any directions of guides at all times.
Dress Code: To help all students feel comfortable, confident, and focused during any sessions, we ask that clothing be simple, practical, and respectful of our shared learning space. A solid color shirt or top that fully covers the chest, back, and shoulders. No prints, neon colors, imagery or crop tops. Jeans, pants, or leggings under dresses. No rips, skin tight or flashy embellishments. Closed shoes only for safety reasons.
If a participant repeatedly ignores expectations or puts anyone's safety at risk, staff will first talk with them and redirect. If the behavior continues, we may contact the parent/guardian and ask that the participant be picked up. Fees will not be refunded in that case.
Photo and Media Release:
During the workshop, staff may take photos and video of participants and their projects. Names are never published alongside images unless you give separate written permission. No compensation is owed for use of images. You may withdraw permission at any time by emailing us; we will stop future use but cannot recall materials already printed or distributed.
Assumption of Risk and Liability Release
STEAM activities may involve hands‑on projects using a variety of age‑appropriate tools, materials, and equipment, as well as movement around the workspace and participation in indoor or outdoor activities. Even with careful planning and supervision, these activities carry inherent risks of minor injury or property damage.
I understand these risks and voluntarily allow the participant mentioned in the form to take part in my selected workshop conducted by Stemlight Prep Academy. To the fullest extent allowed by law, I release Stemlight Prep Academy, including its staff, volunteers, instructors, directors, and venue hosts, from any claims for injury, loss, or damage arising from participation, Except when caused by gross negligence or willful misconduct. I agree to follow all safety rules and to inform staff of any condition that may affect safe participation. I understand that I am responsible for any property damage the participant intentionally causes.
